Termination of Club Membership

I signed up at the (name of club)__ ___on (date)______. Working out at your club over the past days/weeks)__ has been a real pleasure for me. Unfortunately, however, I recently experienced an uncomfortable situation with (name)_____ that has become most uncomfortable, and is getting worse as time goes on.
There was a small incident on (date)_____, which I will briefly explain__. I immediately telephoned the manager to discuss it since I wanted to have this situation resolved. After trying to contact …both the manager and co-manager… unsuccessfully by telephone and after leaving a few messages to contact me, I felt quiet disconcerted and despondent upon receiving no reply. Disappointingly, I did not hear from either of them.
As a result of these events, I am terminating my membership with(name of club)____ immediately and request a refund of ($amount)______.
